Abstract:Objective: To assess the effectiveness and safety of greenlight photoselective vaporization of the prostate (PVP) among the patients with benign prostate hyperplasia (BPH) on or off oral anticoagulation. Methods: Embase, Cochrane Library, WanFang Data, China National Knowledge Infrastructure (CNKI). Newcastle-Ottawa Scale (NOS) was used to assess the quality and risk of bias of each study. Six parameters (operation time, catheterization time, hospitalization time, Qmax, IPSS and postvoid residual volume) from patients on or off anticoagulant therapy were collected. All statistical analyses were conducted with Revman V.5.3 software. Results: Totally, 993 patients from 7 studies were eventually included. There were no significant differences in prostate volume, Qmax, IPSS score and residual urine between the two groups. Perioperative operative time in oral anticoagulants group was longer than that of non-oral anticoagulants group, but the difference was not statistically significant. Meanwhile, IPSS in non-anticoagulants group was significantly reduced by 0.6 as compared with that in anticoagulants group at 3rd month postoperation [MD 0.61, 95% CI (0.19 - 1.04), P = 0.005, I2 = 0]. However, there was no significant difference in other parameters. Conclusion: PVP is an effective and safe option for BPH patients on or off oral anticoagulants.
