Transurethral resection of prostate for bladder outlet obstruction induced by benign prostatic hyperplasia

Abstract:Objective: To investigate the factors influencing the efficacy of transurethral resection of prostate (TURP) for bladder outlet obstruction (BOO) induced by benign prostatic hyperplasia (BPH). Methods: Between Jan. 2015 to Jan. 2018, 95 BPH patients with BOO treated by TURP in our hospital were included in this study. All the patients were divided into two groups according to their outcome: invalid group and valid group. The basic clinical data including disease history, age, residual urine volume, preoperative prostate volume, etc., were analyzed. Univariate and multivariate logistic regression analysis of independent risk factors influencing efficacy was also carried out. Results: Among 95 patients receiving TURP, the efficiency rate was 84.21% (80/95) and the inefficiency was 15.79% (15/95); The univariate analysis showed no significant difference was found in age, diabetes mellitus, duration of disease and operation time, maximum urinary flow rat between the two groups (P>0.05); Preoperative residual urine volume, total prostate volume, detrusor compliance, pressure at the maximum flow, obstruction level between the two groups showed statistically significant difference (P<0.05). The multivariate logistic regression analysis showed that the total prostate volume, the preoperative residual urine volume and the obstruction level were independent risk factors influencing the efficacy of TURP in the treatment of BPH. Conclusion: TURP is an effective treatment for BOO induced by BPH, and the total prostate volume ≥ 50 cm3, preoperative residual urine volume ≥ 200 mL and obstruction level ≥ 3 can influence the efficacy of the treatment.
