Abstract：Objective: To compare and investigate the clinical effectiveness of laparoscopic decortication of renal cysts through the retroperitoneal and transperitoneal approache. Methods: We retrospctively analyzed the data of 35 cases of renal cysts from July 2010 to July 2014 in our hospitals, including 12 cases given the transperitoneal approach and 23 cases given the retroperitoneal approach. Preoperative characteristics, operation time, diet recovery, intraoperative blood loss and hospital stay were documented. Results: All operations were successfully completed. There was no open surgery, no side injury and no complication. The retroperitoneal approach had shorter operation time (P<0.05). There was no significant difference in intra-postoperative blood loss, time of drainage removal and hospital stay between the two groups. Pearson's chi-square test indicated a higher degree of surgeon's satisfaction to the operation procedure, the diet recovery, postoperative complication and postoperative activity in the retroperitoneal approach group than in the transperitoneal approach group (P<0.05). Conclusions: The retroperitoneal approach is a good clinical expiosion. Each procedure of laparoscopic decortication had its merits and demerits. The final decision is individualized and is, in fact, a consensus based upon the location, volume of the cysts as well as the surgeon's skill and proficiency of procedure, etc.
