Abstract:Objective: To investigate laparoscopic pyeloplasty combined with electron cystoscope or ureteroscope for the treatment of ureteropelvic junction obstruction (UPJO) complicated with renal calculi. Methods: Retrospective review of the clinical data of 15 patients with renal stones treated with UPJO between January 2019 and January 2021. With the use of laparoscopy combined with soft endoscope, we can simultaneously fundamentally treat two diseases with the use of minimally invasive technique in the first stage and achieve clinical cure. The dilated renal pelvis and upper ureter was educed intraoperatively with laparoscope. The clipping of dilated renal pelvis was cut 1 cm, electron cystoscope or electron ureteralscope was implanted, stone was removed with a three- or four-sided stone basket under direct visual inspection, and larger stone was removed through holium laser lithotripsy, laparoscopic pyeloplasty was performed using the Anderson-Hynes method. Results:All patients finished the first stage of surgery in this group. Stones were completely removed. One patient was transfered to open surgery due to hepatic injury caused by intraoperative postponing card. The surgical duration was 140~230 (140.03 ± 36.58) min, the postoperative departure time was 24~72 (39 ±17) h, and the hospital stay was(7.0 ± 2.2)d. D-J ureteral stents were removed 8 weeks postoperatively and followed up for 7~21 months. Hydronephrosis were improved significantly in all 15 patients, and low back pain was relieved. No serious postoperative complications occurred. Conclusion: Laparoscopic pyeloplasty combined with electron cystoscopy or electron ureteroscopy for simultaneous treatment of UPJO with renal stone is safe and reliable, with minimal trauma, rapid recovery and high rate of stone clearance, and is worthy of clinical application.
