Abstract:Objective: To investigate the curative effect and safety of percutaneous nephroscopic dual probe ultrasonic intracorporeal lithotriptor in complex upper urinary tract stones. Methods: The clinical data of 263 patients with complex upper urinary tract stones treated with percutaneous nephroscopic dual probe ultrasonic intracorporeal lithotriptor (Cyberwand) in our hospital from May 2012 to April 2015 were reviewed retrospectively, including 159 males, and104 females. There were 107 cases of renal staghorn calculi, 89 cases of multiple renal calculi, and 67 cases of renal with upper ureteral calculi. Results: 262 cases were successfully operated, and only one case failed to establish renal access and was converted to open operation because of floating kidney. 211 patients got stone clearance in stage one, and stone clearance rate of stage one was 80.23% (211/263), 31 patients got stone clearance instage two, and the total stone clearrance rate was 92.02 (242/263), Twenty-one patients were given ESWL for residual stones. Postoperative pyemia in 3 cases was cured by strenthening anti-infection, and bleeding was controled in 6 patients by superselective embolization. No case suffered from serious complications, such as perforation of kidney, pneumothorax, injury to intestines and so on. Conclusions: The dual probe ultrasonic intracorporeal lithotripter device is a safe and efficient tool, with special advantages in increasing the stone-free rate, cutting down extravasation of irrigation fluid, and decreasing renal intrapelvic pressure and absorption of bacteriotoxin, which can effectivelly decrease the incidence of complications and improve operation safety.
