Abstract:Objective: To investigate the clinical efficacy and safety of ultrasound-guided mini-invasive percutaneous nephrolithotomy(MPCNL) in multiple oblique supine position for upper urinary tract calculi with spinal deformity. Methods: Between July 2013 and December 2017, 12 patients with 13 side upper urinary tract calculus underwent ultrasound-guided percutaneous nephrolithotomy in multiple oblique supine position. The intraoperative hemorrhage and the decrease level of hemoglobin at 48 hours after operation were obtained from medical records,the injury of neighboring organs and complication rate were observed after operation. the mean operation time and postoperative hospitalization duration were recorded. The abdomen film and ultrasound were used to reexamine the removal of upper urinary tract calculi after operation. Results: A total ot 14 MPCNLs were performed in 13 side kidneys, including 12 times of single channel MPCNL and 2 times of double channel MPCNL. All the operations were successful with no complications such as severe hemorrhage or neighboring organ injury during and after operations. The mean operation time was (103±47)min, postoperative hospitalization duration was 5.5 d, and the decrease of hemoglobin was(18±6)g/L at 48 h after operations. In postoperative reexamination, the abdomen film and ultrasound showed that 13 side upper urinary tract calculus were totally removed. Conclusion: Ultrasound-guided MPCNL in multiple oblique supine position is safe and effective in treatment of upper urinary tract calculi with severe spinal deformity.
