Accurate transurethral resection of bladder tumor in the treatment of tumors located in lateral bladder wall

Abstract:Objective: To investigate the methodology and efficacy of accurate transurethral resection of bladder tumors (ATURBT) located in lateral bladder wall using the plasma needle electrode. Methods: Fifty-two patients with tumors located in the lateral bladder wall received ATURBT with a needle electrode. Parameters including resection time, obturator nerve reflex, perforation, TNM stage, and post-operation recurrence were analyzed. Results: All patients were successfully performed ATURBT under epidural anasthesia without any obturator nerve blockade. Totally 62 tumors were resected with diameter of 0.5-4.0 cm (mean 1.75 ± 0.56 cm). The resection time of one tumor was 1-25 min (mean11.6±5.7 min). No obturator nerve reflex occurred during operation. The minimal blood loss, and no bladder wall perforation occurred. Precise TNM stages were obtained, including 32% pTa, 55% pT1, and 13% pT2. No recurrence at the resection site occurred during a mean (range) follow-up of 25.6 (3-32) months. Conclusions: ATURBT can successfully resect tumors located in the lateral bladder wall without obturator nerve reflex. The "en bloc" specimen helps to make an accurate pathological diagnosis as well as clinical stage. ATURBT represents an important step in accurate bladder cancer treatment.
