Abstract:Objective:To introduce the technique of and to evaluate the clinical significance of thoracoscopic transdiaphragmatic right adrenalectomy. Methods:We performed thoracoscopic transdiaphragmatic adrenalectomy on a patient with right retrocaval adrenal pheochromocytoma. Under thoracoscopic direct vision, the diaphragm was incised. The subphrenic extraperitoneal space was enterd and expanded. The adrenal tumor was mobilized and excised. Results:The operation was successfully performed with the operating time of 3.5 hours and the estimated blood loss of 50 mL. No drainage tube was placed in the surgical field, and no intraoperative or postoperative complications were documented. Ambulation and oral intake were resumed on day 1, and the patient was discharged on day 4 postoperatively. Conclusions:Thoracoscopic transdiaphragmatic right adrenalectomy is feasible. For retrocaval adrenal tumors, this technique posses the advantages over the transperitoneal or retroperitoneal laparoscopic adrenalectomy.
