Retroperitoneal laparoscopic radical resection of left renal carcinoma with situs inversus totalis: 1 case report and literature review

Abstract:Objective: To observe the posterior abdominal anatomical changes of the situs inversus totalis (SIT) (or visceral translocation) by retroperitoneal laparoscopic radical surgery on the left kidney. Methods: A retrospective analysis was conducted on the diagnosis and treatment of a patient admitted to our hospital in September 2017 who received retroperitoneal radical nephrectomy of the left kidney accompanied by visceral translocation of a left renal carcinoma. Combined with the video of the operation, the anatomical changes of posterior abdominal visceral translocation and related surgical strategies were discussed by reviewing relevant literature. Results: The patient was diagnosed as having total visceral translocation complicated with left renal carcinoma. The anatomical structures of inferior vena cava, short renal vein and reproductive vein were visible in the left posterior abdominal cavity. Its anatomic characteristic was opposite to the normal person, namely left renal artery was longer, left renal vein was very short, and at the same time, genital vein and the left adrenal central vein that enters inferior vena cava were seen. Conclusion: The incidence of visceral translocation is relatively low, and retroperitoneal laparoscopic surgery is more rare. Therefore, detailed imaging examination should be conducted before surgery to identify retroperitoneal anatomy and fully consider the anatomical variation structure to avoid injury to these tissues.
