Abstract：Objective: To investigate preoperative factors which can predict the risk of distant metastasis of clear cell renal cell carcinoma in patients after laparoscopic radical or partial nephrectomy. Methods: 124 patients receiving laparoscopic radical or partial nephrectomy were retrospectively analyzed in our hospital between December 2008 to May 2012. All the tumors were confirmed pathologically as clear cell renal cell carcinoma. The clinical features of gender, age, symptoms, preoperative blood pressure, BMI, T stage, clinical stage, size of tumors and Fuhrman stage were analyzed. Cox regression model was applied to analyze the relationship between the clinical features and the occurrence of distant metastasis. Results: Ten of the 124 patients were lost in the follow up and distant metastasis occurred in 29 cases after the surgery. Tumor thrombus, renal carcinoma triad symptom, T stage, BMI, clinical stage, tumor necrosis, size of the tumor and the Fuhrman stage were statistically significant in the univariate analysis (P＜0.05). The multivariate Cox regression analysis revealed that only clinical stage was the independent prognostic factor for distant metastasis in patients after laparoscopic surgery. Conclusions: Tumor thrombus, renal carcinoma triad, T stage, BMI, clinical stage, tumor necrosis, size of the tumor and the Fuhrman stage were the prognostic factors for the distant metastasis of the clear cell renal cell carcinoma in patients after laparoscopic radical nephrectomy, among which clinical stage was the independent prognostic factor.
