Abstract：Objective:To investigate the feasibility of re-operation by retroperitoneal laparoscopy in adrenal region. Methods:Four patients who underwent retroperitoneal laparoscopy for a second operation from January 2011 to January 2014 in our hospital were analyzed retrospectively. All the patients had received an ipsilateral operation by laparoscopic orretroperitoneal laparoscopic approaches. Among the patients, one was diagnosed as having recurrent pheochromocytoma, two as having APA, and one as having UNAH. The interval between the two operations ranged from 0.5 to 4.3 years with a mean of 2.5 years. Retroperitoneal laparoscopic approach was employed in all the patients for the second operation. Through an incision at the midaxillary line 2 cm above the crista iliaca, we introduced the first trocar into the abdominal cavity and established pneumoperitoneum. The lesions were finally removed by anatomically separating the tissues layer by layer. Results:The procedure was completed in all the patients within 75-183 min (mean 101 min). The intraoperative blood loss was 136 mL averagely (ranging from 50 to 300 mL). The patients were discharged from hospital in 4 to 10 days (mean 6.8 days) after thesecond surgery, and all of them received a follow-up for 4 to 42 months and no major complication occurred. Conclusions:Re-operation by retroperitoneal laparoscopy in the adrenal region is feasible in properly selected cases with skilled and experienced hands.
