摘要目的:探讨经脐单孔腹腔镜手术(LESS)行肾上腺切除术的疗效和安全性。方法:2010年4月~2011年2月期间,行LESS肾上腺肿瘤切除术5例。采用紧邻脐孔边缘弧形皮肤切口,穿刺针穿入腹腔建立气腹,于弧形切口穿入TriPort;正中的操作孔进入5 mm腹腔镜,余2个操作孔进入手术器械。评估手术时间、术中出血量、手术并发症、术后镇痛时间、术后住院日等相关指标。结果:5例患者LESS均获成功,其中1例右肾上腺切除术,因暴露困难,于腋前线右肋缘下增加一5 mm Trocar 帮助暴露。术中、术后未发生严重并发症。结论:LESS可安全用于肾上腺切除术,而与传统腹腔镜相比,疗效相当且具有更好的美容效果。
Abstract:Objective:To evaluate the feasibility of laparoendoscopic single-site surgery(LESS) through umbilical single port for adrenalectomy.Methods:We performed 5 umbilical laparoendoscopic single-site adrenalectomy. A single incision was made beside the umbilicus. After the establishment of pneumoperitoneum, a triport was placed in the incision. The 5 mm laparoscopy was introduced through the middle port, and operation instruments were put through the other two ports. Operation time, amount of bleeding, intraoperative complications and postoperative analgesia time, postoperative hospital stay was evaluated.Results:All of the five surgeries were completed successfully. In one case of right adrenalectomy, an additional trocar was employed because of difficult exposure. There were no major intraoperative or postoperative complications.Conclusions:Our results suggest that LESS is a safe and feasible technique for adrenalectomy and has better cosmetic outcomes than conventional laparoscopic surgery.
