Efficacy and safety of 180W laser photoselective vaporization of the prostate in the treatment of benign prostatic hyperplasia with a report experience of 433 cases

Abstract:Objective: To evaluate the clinical efficacy and safety of 180W greenlight laser photoselective vaporization of the prostate (PVP) in the treatment of benign prostatic hyperplasia (BPH). Methods: A retrospective analysis was performed on the patients who underwent 180 W greenlight laser PVP between June 2018 and December 2020. The patients’ data including preoperative data, indicators during hospitalization, postoperative complications and follow-up information were collected. Results: A total of 433 patients were included with a age of (69.6±8.7)years. Preoperative prostate volume was (68.8±28.5)ml. The operation time was (53.2±23.8)min. The mean bladder irrigation time was (17.3±3.2)h and mean catheterization time was (23.3±6.8)h. With a mean postoperative hospital stay of (1.6±1.0) days, the rate of postoperative urinary retention, retrograde ejaculation and urethral stricture was 5.3%, 0.9% and 0.9%, respectively. No patient had blood transfusion, unplanned reoperation or death. The International Prostate Symptom Score (IPSS) of patients decreased significantly from (21.1±3.3) at baseline to (9.5±3.7) at 1month, (77.0±3.3) at 3 months, (6.9±2.3) at 6 months and (7.1±2.5) at 12 months. Meanwhile, the quality of life (QOL) score was also significantly reduced from (4.9±0.6) at baseline to (2.9±0.7) at 1 month, (1.8±0.9) at 3 months, (1.8±0.9) at 6 months and (1.8±0.6) at 12 months. Conclusion: 180W green laser PVP for the treatments of BPH is safe and effective with the advantages of less bleeding, quicker postoperative recovery and fewer complications.
