Effect comparison of vacuum suction ureteral access sheath and normal ureteral access sheath in flexible ureterorenoscope lithotripsy

Abstract:Objective: To compare the effect of vacuum suction ureteral access sheath and normal ureteral access sheath in flexible ureterorenoscope lithotripsy. Methods: Compare clinical data of patients underwent flexible ureterorenoscope lithotripsy between 2017-08 and 2019-12. 100 patients were randomly allocated in each group, respectively. Intraoperative situation, inflammatory indicators and stone residual rate were observed and compared. Results: Vacuum suction sheath group showed advantages over normal sheath group in most observed aspects with statistical significance.Intraoperative situation: operation time (44.91±16.54 vs.54.20±19.78 min), ureter injury (0 cases vs.2 cases); Inflammatory indicators: instant WBC count (10.3±4.2×109/L vs.12.8±5.0×109/L), instant PCT (1.64±1.22μg/L vs.2.43±3.33 μg/L), WBC fallbackduration (1.23±1.07 days vs.1.72±1.14 days), PCT fallback duration (1.26±0.96 days vs.1.68±1.11 days), peak temperature (37.19±0.95℃ vs.37.78±1.17℃), qSOFA≥2 (2/9 cases); Stone residual rate1 day (13% vs.27%), stone residual rate 30days (3% vs.9%). Septic shock did notshow statistical significance, but both trend and case number revealed vacuum suction sheath has the advantage. Conclusion: Vacuum suction ureteral access sheath has great advantages in direct sheath insertion, vacuum suction of irrigation liquid and stone fragments, low ureter injury rate and low stone residual rate.
