输尿管镜术后发生泌尿道感染的危险因素分析

摘要 目的:探究输尿管镜术后泌尿道感染发生的危险因素。方法:回顾性分析2010~2016年接受输尿管镜检查或手术的236例患者的临床资料,收集患者的年龄、性别、实验室指标、抗菌药物使用情况等资料,统计泌尿道感染的发生率及其影响因素。结果:236例患者中,共有19例患者发生术后感染,发生率为4.4%。不同类型药物使用的感染率相比,差异无统计学意义(P>0.05)。年龄、糖尿病、术前尿潴留、预防性使用抗菌药物、术后留置尿管时间是输尿管镜术后泌尿道感染发生的相关因素(P<0.05)。糖尿病、术前尿潴留和留置尿管时间较长是输尿管镜术后泌尿道感染的危险因素,预防性使用抗菌药物则是其保护因素。结论:术前尿潴留、合并糖尿病以及术后长时间留置尿管会导致输尿管镜术后泌尿道感染率升高,而预防性使用抗感染药物则能够降低感染的发生率,临床应用过程中应注意这些问题,尽量避免泌尿道感染的发生。

Abstract：Objective:To explore the risk factors of urinary tract infection after ureteroscopy. Methods:The clinical data of 236 cases from 2010 to 2016 in our hospital undergoing ureteroscopy or surgery were retrospectively analyzed, including patients' age and sex, complications, laboratory indexes, antibiotic use, incidence and risk factors of urinary tract infection, etc. Results:Of the 236 patients, there were 19 cases of postoperative infection with the incidence being was 4.4%. There was no significant difference in the infection rate among different types of drugs used (P>0.05). Age, diabetes, preoperative urinary retention, prophylactic use of antibiotics and postoperative indwelling catheter time were related factors of urinary tract infection after ureteroscopy (P<0.05). Diabetes, preoperative urinary retention and long indwelling catheter were risk factors of urinary tract infection after ureteroscopy. Preventive use of antibiotics was the protective factor. Conclusions:Preoperative urinary retention, diabetes mellitus and postoperative long time indwelling catheter will lead to higher rate of urinary tract infection after ureteroscopy, and prophylactic use of anti-infective drugs can reduce the incidence of infection.
