Abstract：Objective: To compare the difference in clinical effectiveness of laparoscopic adrenalectomy via the transperitoneal approach vs. retroperitoneal approach. Methods: Of the 124 patients subject to laparoscopic adrenalectomy selected from January 2011 to December 2013, 88 patients were fallen into transperitoneal approach group, and 36 patients into retroperitoneal approach group. The operation situation, conversion rate and complication rate were compared between the two groups. Results: Through the right approach, the operation time (55.5±12.3 min vs. 59.2±10.1 min), blood loss (63.2±21.4 mL vs. 73.1±26.6 mL), intestinal recovery time (1.4±0.2 days vs. 1.5±0.4 days) showed statistically significant difference between two group, while there was no significant difference in conversion rate (2.70% vs. 5.88%) and complication rate (5.41% vs. 11.76%). Through the left approach, there was no significant difference in conversion rate (0 vs. 526%) and complication rate (3.92% vs. 10.53%) between two groups. Conclusion: The patients subject to laparoscopic adrenalectomy via the right transperitoneal approach had a shorter operation time, less blood loss and faster intestinal recovery time, while those via the left retroperitoneal approach had a faster intestinal recovery time and shorter hospital stay.
