Middle-term and long-term follow-up of retroperitoneal laparoscopic pyelolithotomy in the treatment of renal pelvis calculi

Abstract:Objective: To explore the middle-term and long-term follow-up effects of retroperitoneal laparoscopic pyelolithotomy (RLP) in the treatment of renal pelvis calculi. Methods: Eighty-two patients with renal pelvis calculi treated in our hospital during the period from February 2014 to October 2015 were selected as the subjects. According to different treatment methods, the patients were divided into control group [treated by percutaneous nephrolithotomy (PCNL), n=40] and study group (treated by RLP, n=42). Operation associated indicators and the clinical curative effect were compared between the two groups. Changes in renal function [blood urea nitrogen (BUN), creatinine clearance rate (Ccr), 24 h urine protein quantitation] in the two groups before and after operation were observed, and the incidence of long-term complications was compared between the two groups. Results: There was no significant difference in the operation time between the two groups (P>0.05). The intraoperative blood loss, length of hospital stay and postoperative blood transfusion rate in the study group were significantly less/shorter/lower than those in the control group (P<0.05). After treatment, the total treatment effective rate in the study group was significantly higher than that in the control group (88.10% vs. 70.00%) (P<0.05). There were no significant differences in BUN, 24 h urinary protein quantitation and Ccr between the two groups before operation (P>0.05). After operation, BUN and 24 h urinary protein quantitation were increased significantly while Ccr was decreased significantly only in the control group, and changes in the indexes were more obvious in the control group than in the study group (P<0.05). The incidence of long-term complications in the study group was significantly lower than that in the control group (28.57% vs. 50.00%) (P<0.05). Conclusions: Compared with PCNL, RLP has the advantages of small trauma, fast recovery, good middle-term operation effects, few complications and little damage to renal function in the treatment of renal pelvis calculi.
