Effects of different double J tube indweURLing methods before ureteroscopic holmium laser lithotripsy on the efficacy of upper urinary tract infection caused by ureteral calculi

Abstract：Objective： To observe the effects of different double J tube indweURLing methods of ureteroscopic holmium laser lithotripsy (URL) on the efficacy of upper urinary tract infection caused by ureteral calculi. Methods： The clinical data of 82 patients with ureteral calculi complicated with upper urinary tract infection who were treated with double J tube indweURLing before URL were retrospectively analyzed. According to the double J tube indweURLing methods, the patients were divided into ureteroscopic indweURLing in vivo group (observation group, n=44) and conventional retrograde indweURLing group (control group, n=38). The changes of blood and urine related indicators and related symptoms [white blood cells (WBC), urine WBC, urine β2-microglobulin (β2-MG), blood procalcitonin (PCT), urine culture, fever, lumbago] were observed before and 7 days after double J tube indwelling in the two groups. The operation-related data (preoperative double J tube indwelling time, operative time, stone emptying time, postoperative hospital stay) and incidence of postoperative complications were compared between the two groups. Results： At 7th day after double J tube indwelling, the blood WBC, urine WBC, urine β2-MG, and blood PCT in the two groups were lower than those before indwelling, and the decline in the above indicators in the observation group was greater than that in the control group (P < 0.05). There was no significant difference in the positive rate of urine test, incidence of fever and lumbago between the two groups (P > 0.05). The preoperative double J tube indwelling time and incidence of urinary irritation in the observation group were lower than those in control group (P < 0.05). There were no significant differences in the operative time, stone emptying time, postoperative hospital stay and incidence of hypothermia, fever, and ureteral perforation between the two groups (P > 0.05). Conclusions： Ureteroscopic indwelling double J tube in vivo has better control effects than conventional retrograde indwelling for upper urinary tract infection caused by ureteral calculi. And the former one is beneficial to the early implementation of URL and postoperative recovery.
