Clinical efficacy of modified laparoscopic radical prostatectomy with nerve sparing

Abstract:Objective: To investigate the clinical efficacy of modified laparoscopic radical prostatectomy with nerve sparing in the treatment of localized prostate cancer. Methods: A retrospective analysis was performed on 78 patients with prostate cancer diagnosed from June 2017 to March 2020 who were younger than 70 years old, staged T1-2N0M0, Gleason score ≤ 8, and had erectile function before surgery. According to the surgical procedures, modified nerve-sparing surgery or standard radical surgery, the patients were divided into the modified nerve-sparing surgery group (n = 36) and the standard radical surgery group (n = 42). The operative time, intraoperative blood loss, drainage tube retention time, catheterization duration, days of postoperative hospital stay, positive rate of pathological margin, the incidence of urinary incontinence and IIEF-5 score at 6th month after operation were compared between the two groups. Results: There were no significant differences in operative time, intraoperative blood loss, drainage tube retention time, catheterization duration, days of postoperative hospital stay, positive rate of pathological margin between the two groups (P > 0.05). The incidence of incontinence in the modified group was significantly lower and IIEF-5 score was significantly higher than those in the standard group (P < 0.05). Conclusions: Modified laparoscopic radical prostatectomy with nerve sparing not only has little injury and quick recovery, but also can protect the continence and sexual function of patients to a great extent. The modified laparoscopic radical prostatectomy with nerve sparing has satisfactory clinical effect and is worthy of popularization and application.
