Effects of super-mini and standard percutaneous nephrolithotomy on renal function and renal hemodynamics of patients with middle and upper caliceal calculi

Abstract：Objective： To investigate the effect of super-mini and standard percutaneous nephrolithotomy （PCNL） on renal function and renal hemodynamics of patients with middle and upper caliceal calculi. Methods： Totally 90 patients with complicated middle and upper caliceal calculi admitted to the urology department of our hospital from January 2017 to December 2019 were enrolled. They were divided into standard group and super-mini group by random number table method （n=45 each）. The patients in the standard group were treated with standard channel PCNL, and those in the super-mini group were treated with super-mini PCNL. The perioperative data of two groups were compared. The changes of renal function and renal hemodynamics were observed before and after operation. Results： There was no significant difference between two groups in the one-time stone clearance rate and postoperative complications （P > 0.05）. The operation time, peak systolic flow rate （Vmax） and end-diastolic flow rate （EDV） was significantly increased, and the reduction of Hb, hospitalization time, serum creatinine （Scr）, blood urea nitrogen （BUN）, urine kidney injury molecule-1 （KIM-1） and renal artery resistance index （RI） were significantly decreased in the super-mini group as compared with those in the standard group （P < 0.05）. Conclusion： PCNL with both super-mini channel and standard channel has good curative effect in the treatment of middle and upper calyx calculi, but super-mini channel has little effect on renal hemodynamics and renal function, which has better clinical application value.
