Abstract:Objective: To investigate the effect of transurethral holmium laser in the treatment of middle and lower ureteral calculi. Methods: The clinical data of 330 cases of ureteral calculi treated with holmium laser lithotripsy in our hospital from August 2013 to May 2015 were analyzed, including 212 cases of lower ureteral calculi, and 118 cases of middle ureteral calculi. Among all cases, 61 patients underwent extracorproeal shock wazve lithotripsy but failed. Under the guidance of zebra urological guidewire, the direct insertion method or 180 degree turnover insertion method, the ureteroscope could be smoothly inserted into the ureter among all cases. Using "eating method" or "drilling method" , under the microscope, with the holmium laser fiber head as the reference, the stone was gradually broken down to about 2 mm. Results: In this group, 322 cases were successfully treated and 8 cases failed. The operation time was 25 to 110 min (average 45.23 min) and hospitalization time was 5 to 9 days (mean 5.6 days). Forty-five cases had gross hematuria, 12 cases had mild fever, and symptoms disappeared after symptomatic treatment. The stone clearance rate was 94.55% (312/330) at 4th week after operation and 100% at 6th week of follow-up. Conclusions: Transurethral holmium laser lithotripsy for ureteral calculi was safe and reliable with definite effect, less trauma, quicker recovery, shorter hospitalization time. Mastering the method and techniques of ureteroscope can improve the efficiency and the success rate of lithotripsy.
