Abstract:Objective:To investigate the clinical safety and feasibility of robotic transumbilical laparoendoscopic single-site renal cyst decortication. Methods:From December 2013 to January 2014, 3 cases of robotic transumbilical laparoendoscopic single-site renal cyst decortication were performed in our department. The patient was placed in a 45° flank position for transperitoneal surgery. A 2-cm skin incision around the umbilicus was made and deepened to the anterior rectus fascia, then the peritoneum was incised and the Port was deployed. A 8.5-mm video-laparoscope, two 5-mm trocars and a 5-mm assisted trocar were inserted through the Port system. Diagnostic laparoscopy was performed before robotic checking. The operative robot then was brought in over the patient' healthy side. Standard procedure of laparoscopic renal cyst decortication was performed. The specimens were removed through the umbilical incision. Results:Three cases of robotic transumbilical laparoendoscopic single-site renal cyst decortication were performed successfully with no conversion to open or 3-port laparoscopic approach. The operative time was 75, 65 and 50 min, the estimated blood loss was 20, 15 and 10 mL, and the postoperative hospital stay was 2, 3 and 3 days, respectively. No intro-operative complication was observed. There was no postoperative complications during a short follow-up period. Conclusions:Initial experience on robotic transumbilical laparoendoscopic single-site renal cyst decortication had shown its clinical safety and feasibility. However, those results should be further proved by large sample, multiple centers, random control study and long-term follow-up.
