Application of ERAS concept in perioperative period of laparoscopic radical cystectomy with orthotopic ileal neobladder

Abstract:Objective: To investigate the applied value of Enhanced Recovery After Surgery (ERAS) concept in laparoscopic radical cystectomy with orthotopic ileal neobladder. Methods: The clinical data of 70 cases who accepted laparoscopic radical cystectomy with orthotopic ileal neobladder during October 2016 to December 2019 were retrospectively analyzed, and all patients were divided randomly into the control group and the observation group. The control group was given concept management of conventional treatment during perioperative period, and the observation group was given ERAS concept management during perioperative period. The operations of the two groups were performed by the same surgeons. The perioperative situation and postoperative recovery and postoperative QoLs of the two groups were compared. Results: The first feeding time, the first time of exsufflation, the first stool time, the first time out of bed, the postoperative drainage tube removal time, the postoperative hospitalization time, the hospitalization cost and the postoperative complication rate in the observation group were significantly better than those in the control group (P < 0.05). The QoLs in the observation group was significantly higher than that in the control group (P < 0.05). There was no significant difference between the two groups in the volume of bleeding during operation and the operative time(P < 0.05). Conclusion: Application of ERAS concept in laparoscopic radical cystectomy with orthotopic ileal neobladder can promote the postoperative recovery of patients and sharply reduce the hospitalization cost with low postoperative complications rate, which is worthy of further application and extension.
