肾绞痛首次发作后不同时间段碎石治疗输尿管中下段结石临床对比研究

摘要 目的:探讨肾绞痛首次发作后14 d内和14 d后行碎石手术治疗输尿管中下段结石临床疗效及安全性差异。方法:研究对象选取2011年5月~2016年5月收治行输尿管镜碎石术输尿管中下段结石患者共90例,其中于肾绞痛首次发作后14 d内行手术治疗者共48例,设为A组,于肾绞痛首次发作后14 d后行手术治疗者共42例,设为B组,比较两组患者手术时间、一次性碎石成功率、术前感染发生率,输尿管息肉发生率及术后2周双J管拔除率等。结果:B组患者手术时间显著长于A组,差异有统计学意义(P<0.05);两组患者一次性碎石成功率比较差异无统计学意义(P>0.05);B组患者术前尿常规检测感染发生率显著高于A组,差异有统计学意义(P<0.05);两组患者术前尿培养检测感染发生率比较差异无统计学意义(P>0.05);B组患者输尿管息肉发生率显著高于A组,差异有统计学意义(P<0.05);B组患者术后2周双J管拔除率显著低于A组,差异有统计学意义(P<0.05)。结论:输尿管中下段结石患者肾绞痛首次发作后14 d内行输尿管镜碎石术治疗可有效缩短手术时间,降低术前感染和输尿管息肉发生风险,且有助于减少术后留置双J管时间。

Abstract：Objective:To investigate the clinical effects and safety in different time range including within 14 d and 14 d after first episode of renal colic by ureterscopic lithotripsy in the treatment of ureteral calculus. Methods:Ninety patients with ureteral calculus by ureterscopic lithotripsy were chosen from May 2011 to May 2016 in our hospital and divided into two groups according to presenting time with their first episode of ureteric colic: group A (n=48) and group B (n=42). The operation time, once disposable stone clearance rate, the infection incidence before operation, the occurrence rate of ureter polyp and double J tube indwelling rate in 2 weeks after operation were compared between two groups. Results:The operation time in group B was significantly longer than in group A (P<0.05). There was no significant difference in the once disposable stone clearance rate between two groups (P>0.05). The infection incidence before operation by urine analysis in group B was significantly higher than in group A (P<0.05). There was no significant difference in the infection incidence before operation by urine culture between two groups (P>0.05). The occurrence rate of ureter polyp in group B was significantly higher than in group A (P<0.05). The double J tube indwelling rate in 2 weeks after operation in group B was significantly lower than in group A (P<0.05). Conclusions:Ureterscopic lithotripsy in the treatment of ureteral calculus in 2 weeks after the first episode of ureteric colic can efficiently shorten the operation time, reduce the risk of infection incidence before operation and ureter polyp, and be helpful to decrease the double J tube indwelling time after operation.
